2.	E-mail one, recent, clear-picture to be used for prayer cards to touchinglivesforchristyahoo.com
3.	Pay the Application Fee of $25.00. (Married couples only pay one fee.) The application fee can be paid online using Paypal.

	Team leaders must also fill out the Team Leader Application. Team leaders must also print out three copies of the Confidential Reference Questionnaire and have them filled out by acquaintances.
